the following background questions. Providing accurate and
complete background information about yourself is important.
The information that you provide will be kept confidential.
If you register on the Internet, you will answer the following
questions interactively as you register. If you register
by U.S. mail, enter your responses to the following questions
on the registration form. Each question number corresponds
to the section of the registration form where you will record
your response.

NYCDOE Program

If you are associated with a New York City Department of Education (NYCDOE) program and
have been directed by the NYCDOE to report your test scores to them, indicate the NYCDOE
program from the list below that applies to you. If you are registering by mail, enter the two-digit
program code that corresponds to your program on the registration form. Select only one
program.
If you select an NYCDOE program, your test scores will be released to the NYCDOE as well as
the NYSED and the institution you indicate in 13, if applicable.

If you are currently matriculated (enrolled
in a degree program) or have been matriculated
at any time since September 2, 1993, at a New
York State college or university, indicate that college or university. If you attended more than one institution
on the list, please select the
institution that granted or will grant the more
advanced degree. If you are registering by mail, see “Institution Codes” for the three-digit institution code to enter on the registration form.
Your test scores will be released to the New York State institution that you indicate during
registration unless you provide written notification to Evaluation Systems that you do not wish
your scores to be released to that institution. Your written notification must be received by
Evaluation Systems by one week after the test date. |
